Questions before a first visit
Do I need to fill in a form before I come?
Request an appointment, then fill in the new patient registration form before you come in. It saves you time at the front desk. The form works on a computer or phone, not on a tablet.
What should I bring?
A photo ID, your insurance card, a list of the medications you take, and any X-rays or MRI images you already have.
Where will I be seen?
At 2011 Murphy Ave, Suite 603, Nashville, TN 37203, Monday to Friday, 8am to 5pm. Fridays with Dr. Jones: 6130 Nolensville Pike, Nashville, TN 37211.
I'm coming from out of town.
Patients travel to Nashville from across the country and abroad. We plan the trip with you, so most surgery can be done in three days. PAO and hip preservation patients usually need more work-up than one trip allows. Some abductor repairs can be done in a single trip.
